POLICIES

  • We require 24 hours' notice for any changes or cancellations to an appointment.  First no show/last minute cancellation will result in a warning. Failure to provide adequate notice for the second time will result in a fee of $40 in order to book any more appointments. If your appointment is on Tuesday at 1pm we ask that you cancel/reschedule no later than Monday 1pm.  This will allow us enough time to fill your slot with another patient who requires an appointment.

  • Multiple missed appointments without any reason will result in dismissal from our clinic.

  • It is the responsibility of the parents to make sure contact information and health card details are up to date. Please also make sure to bring the health card to each visit.

  • Please arrive on time for your appointments unless advised otherwise. 

​

  • Certain services which are part of your child's care including medical record transfer, disability tax form filing, notes for daycare/school are not covered by OHIP and therefore will be charged in accordance with Ontario Medical Association guidelines for Uninsured Services. This is payable in cash or via etransfer and must be paid when the service is provided.

  • We are not an urgent care facility. If your child is in need of immediate medical attention, please go to the nearest emergency. However, South Mackenzie Kids Clinic will have availability of same day sick visits and we will do our best to accomodate patients for urgent appointments. 

​

  • Abusive language and/or behaviour towards physicians ands staff will not be tolerated and is grounds for dismissal from our clinics.
